Need a ride to your next medical appointment? Your Medicaid card is the best place to start. Your transportation options depend on whether you are enrolled in an HMO or have “Straight” Medicaid.
If You Are Enrolled in an HMO:
A Health Maintenance Organization (HMO) is a network of doctors and staff coordinated to provide your care. These organizations are contracted by the State of Michigan to handle your medical transportation.
How to get a ride: Check your Medicaid card for a member services phone number.
Next steps: Call that number directly. The HMO staff will help you coordinate and schedule your transportation.
If You Have “Straight” Medicaid (No HMO):
If you are not assigned to an HMO, your transportation is managed by your local Department of Health & Human Services (DHHS) office. Community Resource staff will help determine what level of service you qualify for and how to book your ride.
Contact your local DHHS office for assistance:
Berrien County: (269) 934-2000
Cass County: (269) 445-0200
Van Buren County: (269) 621-2800

The Healthy Michigan Plan
Are you looking for health care coverage that works for your health and your budget? Now there is an option to help working people in Michigan get healthy — at a cost that works with your budget.
It’s the Healthy Michigan Plan. See if you are eligible at the Healthy Michigan website. For details of coverage, see the Healthy Michigan Plan Handbook.
Non-Emergency Transportation Services: You can get help with a ride if you do not have a way to get to and from a provider visit that is covered by the Healthy Michigan Plan. You must get approval for non-emergency transportation before your visit. If you are in a health plan, contact your health plan if you need transportation services. If you are not in a health plan, contact your local Department of Health & Human Services specialist.
